Bianchi Report (Zoning Letter of Compliance) (PDF)
The fee is $100 or $50 if eligible for renewal. Please submit a written request two weeks in advance of the closing.  Requests with less than two weeks notice will be charged an extra $100 if they can be processed.
Boundary Line Adjustment Language (PDF)The fee is $215.
Building and Zoning Permit (PDF)

Erosion Prevention and Sediment Control Plan Application (PDF) No Fee
2020 RBES Certificate
2020 Owner / Builder Disclosure Statement		The fee is $15 per page to record in the Land Records.
Request for Action Investigation Report (PDF) No Fee

Sign Permit: Permanent (PDF) The fee is $85.

Development Review Board Applications & Forms

Appeal of Zoning Administrator's Determination (PDF)
The fee is $285.

Includes the legal notice fee in the local Seven Days newspaper as well as all recording fees. If, due to time constraints, an applicant chooses to advertise in the Burlington Free Press, the applicant shall be required to pay the entire fee charged by the Free Press.

Continuance of Development Review Board hearings: $100.00 for each continuance requested by applicant.
Conditional Use Application (PDF)
The fee is $285 (or $145 if submitted with Site Plan Application)

Includes the legal notice fee in the local Seven Days newspaper as well as all recording fees. If, due to time constraints, an applicant chooses to advertise in the Burlington Free Press, the applicant shall be required to pay the entire fee charged by the Free Press.

Continuance of Development Review Board hearings: $100.00 for each continuance requested by applicant.
Site Plan Application (PDF)
The residential fee for single family or duplex dwellings is $235.00.

The Commercial/Industrial/PUD/Residential Multi-family fee is $355.00.

The Commercial/Industrial/PUD/Residential Multi-family Amendment (if site plan less than 5 years old) is $275.

Includes all recording fees.

Site Plan Application Administrative (PDF)  The single family or duplex dwellings fee is $125.00.

The Commercial/Industrial/PUD/Residential Multi-Family fee is $155.00.
Site Plan Application for Final Plat (PDF) The fee is $155.00.

Sketch Plan (PDF) 		The fee is $250.
Subdivision: Preliminary Plat (PDF) The base fee is $355 plus:
  • Major residential subdivision fee of $100.00 per lot/unit.
  • Commercial or Industrial or PUD fee of $125.00 per acre or $125 per lot/unit, whichever is greater.
Includes the legal notice fee in the local Seven Days newspaper as well as all recording fees. If, due to time constraints, an applicant chooses to advertise in the Burlington Free Press, the applicant shall be required to pay the entire fee charged by the Free Press.
